Research Abstract |
The purpose of this study was to investigate the prevalence and of periodontal disease and its relationship with type 2 diabetes mellitus (DM). Subjects were 46 patients with type 2 DM (mean age of 60.7 ±9.3). They had been medically controlled at Meikai University Hospital and never received periodontal therapy. As medical examinations of DM, HbA_<1C>, insulin resistance (HOMA-IR), and body mass index (BMI) were recorded. The following periodontal variables were recorded : number of the present teeth, probing depth (PD), bleeding on probing (BOP), and bone loss score. As a parameter of both DM and periodontal disease, serum concentrations of tumor necrosis factor (TNF)-α and high sensitive C reactive protein (hs-CRP) were measured by enzyme linked immunosorbent assay(ELISA). In all subjects of this study, the glucose levels were relatively well controlled, and the mean BMI was under 25. BMI, the concentration of hs-CRP and TNF-α were significantly correlated with HOMA-IR (BMI : p<0.01, TNF-α and hs-CRP : p<0.05). Compared with the results on the survey of dental diseases conducted by the Ministry of Health and Welfare in Japan (1999), the incidents with teeth with PD≧6 mm were higher in the DM patients, whereas number of the present teeth was approximately equal. Although HbA_<1C>, HOMA-IR, and BMI were not correlated with any clinical measurements of periodontal disease, duration of DM was significantly correlated with mean average of PD, BOP, rate of teeth with PD≧4 mm and bone loss score. BOP was significantly higher in the patients with insulin treatment than in those without it. These results suggested that patients with DM should receive periodontal treatment at an early stage.
