Research Abstract

DSM-IVに基づいて診断されたアルコール依存症のうち精神神経症状のはっきりした435例(男性)と健常対照男性(1584例)についてヒトコレシストキニン(CCK)-A受容体遺伝子のプロモーター領域に2箇所の塩基置換(-128のGがT、-81のAがG)とアルデハイド脱水素酵素(ALDH)2の多型を解析した。ALDH2野生型は、アルコール依存症384例、対照男性792例であった。CCK-A受容体多型は、アルコール依存症群で発生頻度が多い傾向にあった(P=0.053)。とくに-81のAがGに変異する頻度は依存症で有意に高かった(P<0.023)。アルコール依存症群の中だけで検討すると、-81Gを持つ個体は、反社会的性格を示し、譫妄、家族歴を有する頻度が高く、発症年齢が低かった。異常のことから、-81G変異は、アルコール依存症の危険因子であると同時に、より、難治性の重症のアルコール依存症を起こすと結論された。ADH2多型は、有意の影響を示さなかった。
CCK-A, B, AB受容体(R)遺伝子ノックアウトマウスに、エタノールの腹腔内投与後、30、60、90、120分でのマウスの覚醒状態を正向反射による数値化で計測したところ、有意差は認めなかった。しかし、2瓶法によるエタノール飲用量の相違を各遺伝子型で比較すると、CCK-AR欠損マウスでエタノール飲用量が増加しており、側坐核のドーパミン2受容体タンパク発現が増加していた。従って、CCK-AR欠損がドーパミン2受容体タンパクを増加させることで、飲酒を促進している可能性が示唆された。
