Research Abstract

虚血性心疾患患者におけるNO・superoxide及びNO合成阻害因子の定量評価
虚血性心疾患患者は、冠危険因子(高血圧、高脂血症、糖尿病、喫煙)の長期暴露によって、酸化ストレスおよびsuperoxideの増加に伴う一酸化窒素(NO)の低下およびNO合成阻害因子の増加を認め、血管内皮障害を惹起し、最終的に動脈硬化へ進展することが考えられている。今回、NO合成阻害因子と虚血性心疾患患者との関連を明かにするため、平均59歳の虚血性心疾患患者およびage matchした正常者における静脈内NO・およびNO合成阻害因子を高速液体クロマトグラフィーとELISAを用いて比較評価した。その結果、正常者に比べ、虚血性心疾患患者において、ほとんどの症例で冠危険因子(高血圧、高脂血症、糖尿病、喫煙)の存在を認め、末梢血中の中性脂肪の増加、HDLコレステロールの低下、NOの低下、NO合成阻害物質asymmetric dimethylarginine, ADMAの増加や酸化ストレスの指標となる酸化LDLの増加、NO合成酵素補酵素tetrahydrobiopterin, BH4の低下を認めた。以上の結果より、統合的に評価すると、虚血性心疾患患者は、ATP静注による冠血管予備能の低下を認めたが、冠危険因子の存在に伴う酸化ストレスおよびsuperoxideの増加がNOの産生低下およびNO合成阻害因子を増強し、血管内皮障害を引き起こし、冠血管予備能低下の原因となることが窺われた。
