Research Abstract

母乳性黄疸の原因となる乳児側の原因についてビリルビンUDP-グルクロン酸転移酵素遺伝子(UGT1A1)の解析と黄疸の強度と検討を終了した。UGT1A1の変異は、体質性黄疽であるCrigler-Najjar症候群I型、II型およびGilbert症候群を引き起こす。合計90名の母乳栄養に起因した遷延性黄疸(母乳性黄疸)のUGT1A1の解析結果より、日本人における母乳性黄疸の最も主要な原因がG71R変異であることが判明した。また、新たにUGT1A1の上流のphenobarbital responsive enhancer module (PBREM)に存在するT-3279G多型もこれら母乳性黄疸児に認められ、この多型ももう一つの母乳性黄疸の原因変異であることを発見した。これにより、母乳性黄疸のUGT1A1の変異による原因を明らかにできた。また、強い母乳性黄疽を示す乳児はG71Rのホモ接合体であることがわかった。G71R変異は思春期以降に発症するGilbert症候群の原因であり、これら乳児はGilbert症候群の乳児期の表現型であることが判明した。今回の研究成果を基に、著しい高ビリルビン血症を示す場合、UGT1A1を調べることにより、核黄疸を引き起こすCrigle-Najjar症候群と鑑別でき、安全に母乳性黄疸が管理できることを示すことができた。
また、UGT1A1の遺伝子多型解析をすすめていたところ、Gilbert症候群の主要な発症原因がUGT1A1のT-3279GとA(TA)7TAAの2つの多型がリンクすることが必要であることを発見した(Maruo Y, et al. Human Genetics 2004)。さらに母乳性黄疽との鑑別で、重度の新生児の遷延性黄疸の原因診断のためUGT1A1を調べたところ、Crigler-Najjar症候群I型の新規変異を発見した(Maruo Y, et al. J Pediatr Gastroenterol Nutr 2003;Maru Y, et al. Clin Genet 2003)。
UGT1A1はビリルビンだけでなく薬剤代謝・解毒の第2相を司る重要な酵素である。また、UGT1A1の多型解析に引き続きUGT1A1に隣接するUDP-グルクロン酸転移酵素(UGT1A3とUGT1A4)の遺伝子多型を調べたところ日本人には固有の遺伝子多型が存在し、それが様々な薬剤の代謝・解毒活性に影響することを発見した(Iwai M, et al. J Hum Genet 2004;Mori A, et al. Drug Metab Dispos in press)。
