Research Abstract

高齢社会を迎え、摂食・嚥下機能の低下が大きな社会問題となっている。「摂食・嚥下障害」というとこれまでは中枢性の疾患のみにスポットがあたり、口腔機能の低下からくるものはあまり問題にされてこなかった。しかし実際の介護の現場、また日常生活を営んでいる患者でさえ、「むせる」「飲み込みづらい」などの黄色信号を発しているものが少なくない。摂食・嚥下の一連の動作には直接的に30以上の筋が関与しており、それらが実にタイミングよく連動する。それぞれの筋線維特性を明らかにすることが、これまでの神経生理学的な数々の報告とあいまって、摂食・嚥下の複雑なメカニズムの解明に一助をなすと考える。さらには、これら筋群のほとんどが口腔内、外より明視的に位置を同定することができ、摂食・嚥下障害の医療現場に対しての情報提供にもつながるのではないかと考えた。
そこで、下記2点について重点的に検討を行った。口腔機能特有の現象である離乳期における筋線維特性の変化と、無歯顎状態で機能するマウスの筋線維特性を正常マウスと比較検討した。
1.離乳前後におけるマウス咬筋浅層、深層の筋線維特性の変化
2.成長期のmicrophthalmiaマウスを用いた咬筋筋線維特性の検索
さらには、舌筋、顎二腹筋など他の筋に関する離乳期の筋線維特性、in vitroという条件にメカニカルストレスを与えた実験などで、機能と筋線維特性の変化を詳細に検討した。今回研究機関の間で、離乳など筋の機能が劇的に変化するタイミングで、多くの筋組織でMHC2aというIsoformが増加していることが明らかとなった。また、このIsoformは無歯状態の筋などでは、優位に少なかった。よって、筋機能低下に最も関連がある重要なIsoformであることが示唆された。
