Research Abstract

癌の治療効果判定は様々な治療が行われた後にCTやMRI等の形態学的評価で行われている。しかし、その効果を治療中に予測することは形態学的検査では難しい。一方、PET検査は代謝診断薬を用いるため細胞の代謝の評価が容易で、例えば^<18>F-FDGでは癌細胞の糖代謝の測定が定量的に可能である。乳癌や肺小細胞癌患者の化学療法前、化学療法1コース後に^<18>F-FDG PETを施行し、^<18>F-FDG PET検査が治療効果予測に有用か否かを検討した。対象は進行乳癌7例、肺小細胞癌9例である。方法は治療前後に^<18>F-FDG PETを施行し、定量的評価として腫瘍領域に関心領域を設定し、単位体重あたりの投与量に対する集積比であるstandardized uptake value (SUV)を算出した。治療前のSUV値をSUV_<1st>、治療中のSUV値をSUV_<2nd>として治療の変化率(SUV_<1st>-SUV_<2nd>)×100/SUV_<1st>を求め、抗癌剤に対する治療の反応を組織学的評価あるいは経過観察から反応群と非反応群に分け検討した。結果、乳癌の反応群は5例、非反応群は2例、肺小細胞癌の反応群は8例、非反応群は1例であった。反応群のPETでの変化率は70%、非反応群の変化率は10%で両者に有意差が得られた。この結果から、乳癌や肺小細胞癌症例では治療途中に^<18>F-FDG PETを施行することで、抗癌剤の治療効果予測が早期に可能なことが示された。そして、PET検査で測定される代謝変化はCTやMRI等の形態学的変化よりも早期に起こることが示された。
