Research Abstract

ロ腔外科領域では、唇顎口蓋裂患者の顎裂部などの骨欠損に自家骨移植を行っているが、健常部に侵襲を加えるなど、解決すべき点が多い。これを材料で補填する場合、口腔内は易感染性であるため、異物として残存しない吸収性材料が安全であるが、従来型では骨形成能がなく、自家骨への置換に時間がかかった。この問題を解消するため、生体内吸収性を有するβ-リン酸三カルシウム(β-TCP)に骨増殖因子(TGF-β1)を含んだゼラチンハイドロゲルを複合させ、骨再生の足場と薬物送達システム(DDS)を兼ね備えた新規ハイブリッド型の骨補填材の開発を進めてきた。
実際の骨欠損に作製した材料を移植し、その後の変化を観察するため、ラットの頭蓋骨に形成した欠損の骨再生をマイクロフォーカスX線CT装置と骨塩量測定装置により評価した。非移植群では、骨欠損形成後およそ6週目から骨再生が観察され、CT画像では欠損部骨の左右両側の断端部より枝状に矢状縫合部に向う不透過像として確認された。また新たな評価として、医療用画像解析ソフトにより、CT画像の多断面再構成、3D画像の再構成を行った結果、脳硬膜側よりも頭頂側で骨形成能が高かった。また経時的に骨塩量の増加を認めたが、12週目においても骨欠損部は線維性組織の侵入により、完全な自家骨修復は観察されなかった。開発材料の移植群では、材料の吸収に伴いTGF-β1が徐放され、骨再生に要する時間が非移植群に比較し短縮される可能性が示唆された。またゼラチンの化学架橋度を上げることにより、吸収速度は遅延し、TGF-β1の徐放期間を一定の期間延長できることが示唆された。材料の作製にあたってはβ-TCPにゼラチンを複合させると強度が増し、組織再生の有効な足場となるが、その過程において偏りが生じることがあるため、性質を安定させるためには、これを均一に複合させることが重要と思われた。
