高齢者の熱中症予防の検討：体温上昇時に起こる過換気と脳血流低下に着目して

Outline of Annual Research Achievements

本年度は、暑熱下運動時の体温上昇に伴う過換気および脳血流低下反応に及ぼす動脈血中二酸化炭素分圧の影響について検討した。健常男性を対象とし、室温38℃、湿度50%の暑熱環境下において、中強度 (最高酸素摂取量の50%) の一定負荷自転車運動を実施した。運動時において、食道温が安静時よりも1.0、 1.5、 2.0℃上昇し、かつ呼気終末二酸化炭素分圧が運動5分目の値よりも低下した場合に、吸気に100%の二酸化炭素を加え、低下した呼気終末二酸化炭素分圧を運動5分目の値に一時的に戻した。深部体温上昇時の換気反応を検討したところ、いずれの深部体温レベルにおいても二酸化炭素吸入前よりも吸入後に換気量が有意に増加した。さらに、二酸化炭素吸入前の中大脳動脈血流速度は深部体温上昇に伴い徐々に低下し、一時的な二酸化炭素吸入によって、いずれの深部体温レベルにおいても脳血流速度の低下は有意に抑制された。これらの結果から、暑熱下持続的運動時において、深部体温上昇に伴う過換気反応は動脈血中二酸化炭素分圧の低下によって抑制されていること、さらに体温上昇時の脳血流低下反応は動脈血中二酸化炭素分圧の低下によって大部分が説明できることが示唆された。
