Research Abstract |
「objective」 Functional MRI (fMRI) image has a advantage to visualize the localization of brain function non-invasinely with spatial and temporal resolution. Functional MRI also shows a clear enhancement of blood flow by various motion of the extremities. The image contrast of fMRI depicts blood oxy-hemoglobin level dependent changes in brain. We tried to investigate the enhancement of blood circulation in brain by precise movements of fingers and the change of dynamic blood flow in the chronic pain syndrome. 「Methods」 (1) Blood flow enhancement (BOLD method) was observed in five healthy volunteers using fMRI (1.5 T SIGNA LX, GE) with motion tasks of voluntary opposition movements of thumb and fingers. The task of opposition was preformed repeatedly for 10 seconds followed by 10 seconds-rest on fMRI. Data analysis was performed using SPM 96 software. (2) Change of blood flow in brain under motion task and sensory stimuli was analysed in patients with cervical spondylotic myelopathy and fibromyalgia. 「Results」 (1) Brain activation was observed in the contralateral sensory-motor cortex by opposition movements. Supplementary motor area and pre-motor area were also activated with less intensity. (2) In spondylotic myelopathy and fibromyalgia patients, chronic and diffuse pain affected blood circulation in brain. 「conclusions」 The finding mentioned above represents an important role of opposition movements of thumb and fingers for blood circulation in the brain. Functional MRI is a valuable tool to analyze the sensory-motor area of brain as well as to evaluate the effect of rehabilitation.
