Research Abstract

1.Morphological factors of cerebral aneurysms including size, dome/neck ratios, and access routes and occlusion rate of the aneurysms after coil embolization were evaluated by 3DDSA. These factors and experience of operators were assessed and their relationships to the occlusion rates were statistically analyzed. Among these factors, dome/neck ratios and experiences were statistically collated to the occulusion rates.
2.We reviewed 357 cases with intracranial aneurysms. Eighty-six cases were treated by endosaccular coil embolization, and 230 cases were treated by surgical clipping. A clinical assessment was made by the Hunt and Kosnik (H&K) scale on admission and by the modified Rankin Scale at discharge. Angiographical results, complications, and clinical outcomes were evaluated. Angiographical results of surgical clipping were significantly better than those treated with coils. Good clinical outcomes were observed in 72.3% of coil group and in 78.7% of surgical group. Symptomatic comp … More lications were encountered in 2.4% of coil group and 11.3% of surgical group. There were statistically significant differences between the both groups. There were statistically significant differences between operators. H&K scale, age, and operators are significantly correlated to outcomes (P<0.05). Rerupture rate was similar in both groups.
3.To evaluate the postoperative aneurysms less invasively, we evaluated CTA images after clipping compare to 3DDSA images in 20 cerebral aneurysm treated by clipping. In 12 of the 20 aneurysms, CTA can demonstrate the residual aneurysm and neighbouring arteries similarly to 3DDSA. In 5 aneurysms, CTA cannot demonstrate the status of clipped aneurysm due to significant artifact with clips. Materials and numbers of clips affect the image quality.
4.Because performance of microcatheters can affect successful embolization, characteristics of the steam-shaped microcatheters in 11 brands, including shapability, durability, and luminal changes, were evaluated.
