Research Abstract

ヒト気道-肺胞上皮系における幹細胞は"niche cell"として広く理解されつつある。
下気道系の深部粘膜の上皮には前癌病変とされる扁平上皮化生が起こりにくく、同じ下気道でも気管支領域では扁平上皮化生と扁平上皮癌が発生しやすいとの事実がある。肺癌のcommitted precursor cellを探すことは、健常な上皮へのprecursor cellをさがすことでもあるとの仮定を設けた。すなわち、niche cellがbasal cell, goblet cell, ciliated cellへ分化しうるstem cellと考えたわけである。生体の気道上皮がapoptosisによりregenerationを発揮することは生理学的現象である。
癌化を促す外来性ないし内因性刺激がかなりの期間にわたって継続することでbasal cell様上皮となり、ciliogenesisへと向かわずにsquamous metaplasiaに転向し、その刺激が継続されることで発癌にむかうことになると推測した。今年度の特異的成果は、protease activated receptor(PAR)1,PAR2の機能に関して新たな成果が望めるものとなった。PARの活性化により細胞増生、サイトカイン放出、運動性亢進が明らかとなった。PAR2が気道に見出された事実と、気道上皮が皮膚としての潜在能を具備することからこの推測は妥当性が高いと信じられる。好中球浸潤の豊富な慢性気道病変の形成において上皮の剥離と再生が繰り返される。その際新生を促す幹細胞について注目すべき基礎的情報が得られ、剥離と再生を繰り返す中で発生してくる上皮細胞が幹細胞由来であるとみなしたいが、今後ともこの領域での検索を継続したい。
