Research Abstract

II型糖尿病に対して適切な治療を行なわない場合、難治性腎症を併発して末期腎不全に陥る。本研究は、II型糖尿病性腎症の進展因子として腎症初期に観察される糸球体血行障害に注目し、その原因メカニズムとしてアンジオテンシンII(AngII)と酸化ストレスの関与を明らかにすることを目的とした。実験は糖尿病自然発症ラット(OLETFラット)を用いて行い、AT_1受容体拮抗薬であるオルメサルタンあるいはアンジオテンシン変換酵素阻害剤であるテモカプリル(それぞれ餌に0.01%の割合で混合)を、糖尿病発症前の4週齢から11週齢のみの期間に投与し、引き続き通常腎不全を生じる50週齢まで無処置で飼育した。本研究により、OLETFラットでは糖尿病発症前(11週齢)よりすでに、腎臓組織内AngII産生・コラーゲン発現・NAD(P)Hオキシダーゼ発現・酸化ストレスの産生が亢進しており、糸球体血行動態異常が生じていることが証明された。また、重篤な腎症を生じる50週齢でも糸球体血行動態異常ならびに腎臓組織内AngII産生・コラーゲン発現・NAD(P)Hオキシダーゼ発現・酸化ストレスの産生の亢進が継続していることが明らかとなった。また、オルメサルタン・テモカプリルを糖尿病発症前の一定期間のみに投与したOLETFラット群は無処置群と同程度の血糖と血圧を示したものの、蛋白尿および腎臓組織コラーゲン発現は減少し、腎障害は持続的に抑制されていた。また、腎不全による死亡率もこれらの処置により著明に改善された。以上の結果より、OLETFラットでは糖尿病発症前に腎内AngIIと酸化ストレスの活性亢進が生じており、その期間のみにAngIIを阻害すると腎症の進展が持続的に抑制されることが明らかとなった。したがって、II型糖尿病発症前にAngII阻害薬を用いた積極治療により、腎症進展を永続的に抑制できる可能性が示唆された。
