脳卒中発症に関わる遺伝・環境要因相互作用の解明

Research Abstract

一般住民の代表サンプル2000人に対し、24時間自由行動下血圧および家庭血圧測定を実施し、コホート研究により脳卒中発症リスクとの関連及び種々の遺伝・環境要因に関する検討を行った。
1.家庭血圧・24時間血圧と脳卒中発症リスクとの関連について
(1)仮面高血圧の脳卒中リスクは高いが白衣高血圧のリスクは低いことを明らかにした。
(2)夜間血圧下降度減は脳梗塞の、モーニングサージは脳出血のそれぞれリスクとなっていることを明らかにした。
2.遺伝要因と脳卒中発症危険因子との関連について
(1)β1アドレナリン受容体Gly389Arg多型と家庭血圧値および心拍数との関連について検討し、Argアリル保有群で血圧・心拍数が高値であることを発見した。
(2)Urocortin3のR91G多型が白衣効果(随時血圧と家庭血圧値)と関連していることを発見した。
(3)Interleukin10遺伝子多型(-592A/C)が寿命と関連している可能性を明らかにした。
(4)Regulator of G protein signaling-2遺伝子A/G多型が高血圧発症と関連していることを明らかにした。
3.環境要因と脳卒中発症危険因子との関連について
(1)運動不足・喫煙が家庭高心拍の要因であることを明らかにした。
(2)肥満・白衣高血圧が家庭高血圧発症のリスクであることを明らかにした。
