神経芽腫におけるゲノム変化の網羅的検索とそこからの癌関連遺伝子及び予後因子の同定

Research Abstract

小児の代表的な腹部固形腫瘍である神経芽腫では染色体1p36領域の欠失、MYCN癌遺伝子座の増幅、17q領域の増加などの染色体変化と予後との相関が知られているが、その領域からの神経芽腫発症に関わる遺伝子は未だ同定されていない。本研究では、神経芽腫の予後の異なるサブセットに特異的なゲノム変化の同定と、それらの領域からの腫瘍関連遺伝子の同定を目指して、様々な病期の神経芽腫270症例についてアレイCGH解析を行った。本年度は以下に示す結果が得られた。
Genetic subgroupの分類と予後判定用clinical subgroupの同定
アレイCGHによる腫瘍組織のゲノム異常パターンを独自のクラスタリング解析によりグループ化した。Silent, Partial, Wholeの3つに大別した後、神経芽腫で重要とされる1p、11qの欠失と2p上MYCNの増幅の有無からさらに12のサブグループに分けられた(Genetic subgroup)。各グループの予後との関連を検討するため、それぞれのグループのKaplan-Meier解析を行い、その結果から臨床予後による3つのClinical subgroupを設定した。これらの5年生存率は95%(予後良好),76%(予後中間),29%(予後不良)であった。今後はアレイCGHパターンから分類した予後中間群および予後不良群についてさらに詳細に解析を進め、予後と関わっているピンポイントなゲノム異常の同定と、遺伝子発現を組み合わせた解析を行う予定である。
増幅および欠失領域の候補遺伝子の探索
アレイCGH解析結果から、ホモ欠失および遺伝子増幅の候補をそれぞれ174 BACクローン、157 BACクローンに絞り込んだ。これらの候補領域にマップされる遺伝子をUCSCゲノムデータベースより取得し、それぞれについて半定量PCRによる確認実験を開始した。
