ポジトロンCTを用いたアミノ酸輸送担体発現評価による癌治療個別化に関する研究

Research Abstract

本年度で、3年計画の研究課題の3年目が終了した。2年目までに蓄積した症例にさらに症例を加え、F-18標識αメチルタイロシンによるポジトロンCT検査(FAMT-PET)、F-18標識フルオロデオキシグルコースPET(FDG-PET)、X線CT、MRI検査などの画像データと口腔外科領域腫瘍術後症例の病理組織診断とを対比した。これにより、FAMT集積はバイアビリティの高い増殖能に富む腫瘍への集積特異性が高いことが確認され、良性腫瘍や周囲の炎症性組織と考えられる領域にも集積を認めるFDG-PETよりも優れており、特異性の高い腫瘍診断薬であることが確認された。手術症例全例に対して行った抗LAT1抗体による免疫染色では、FAMT集積陽性となった症例の標本では、免疫染色にて、LAT1発現が明瞭に見られたのに対して、FAMT集積陰性の腫瘍では、LAT1発現は見られずFAMT集積とLAT1の発現に相関関係が確認された。LAT1の発現は、細胞増殖マーカーであるKi-67の発現とも相関した。これらの結果を、2005年度オーストラリアニュージーランド核医学会議および2005年度欧州核医学会議にて報告を行った。
3年間の検討で、FAMT集積の程度により腫瘍でのLAT1発現の程度を的確に診断できることが確認できたため、今後、LAT1をターゲットとした分子標的治療の適応決定においてFAMT-PETが役立つものと考えられた。今後、臨床治験などを行う際に、FAMT-PETを行い、臨床例にて有用性の確認がなされることが期待される。
