Outline of Annual Research Achievements

薬剤関連顎骨壊死(MRONJ)に対する画像診断としては、パノラマX線写真・CT・MRI、骨シンチグラフィーによる病変範囲の評価が一般的であるが、北海道大学病院口腔診断内科ではこれらに加えFDG-PETによる画像診断を行っている。一般に顎骨骨髄炎におけるFDG-PETによる評価では、顎骨内に浸潤した白血球等の炎症性細胞に18F-FDGが取り込まれることで、炎症病巣の活動性と範囲を示すことが可能であり、当科ではこの点に着目し、各種骨髄炎に対してFDG-PETによる評価を行っている。MRONJに関しては保存療法の非奏功例に、術前後の高気圧酸素療法(HBO)を伴う外科的消炎術を実施しており、HBO前後でのFDG-PET撮像により、MRONJに対するHBOの消炎効果、また症例ごとの活動性について評価検討を実施した。16名のMRONJ患者に対し、HBO前後の血液検査および各種画像診断を実施し、FDG-PETではSUVmaxに加え、SUV値を三次元に合計したmetabolic volume (MV)やMV とSUVmeanの積であるtotal lesion glycolysis=metabolic activity(MA)をそれぞれ算出し、検討を行った。この結果、血液検査ではHBO前後の変化が認められない一方で、SUVmax、MV、MAにおいて有意な低下が確認され、FDG-PETによるMRONJの活動性の評価が可能であることが示された。この低下はHBOによる血液循環の亢進、白血球での酸素依存性殺菌効果の増強などに伴う抗炎症作用を示したものと考えられる。
