Outline of Annual Research Achievements

職業性ジストニアとは局所性ジストニアに分類され、長期間にわたる過剰な運動学習により、訓練した課題に特異的に異常な不随意運動が生じる神経疾患であるが、その治療法は確立されていないのが現状である。また、Brain-Machine Interface(BMI)とは、機械学習を用いて運動系に関わる神経信号を読み取りロボットハンドなどを制御する、脳と情報装置を直接接続する神経工学技術である。近年、BMIは神経補綴だけでなく、リハビリテーションにも応用されており、治療法が未確立な疾患への応用が期待されている。本研究では、fMRIにより計測された脳活動をリアルタイムに可視化して提示し、その情報を手掛かりに自身の脳活動を調整することで、脳活動や運動機能の改善をはかるreal-time fMRIニューロフィードバックの構築を目的とする。本年度は、音楽家のジストニアを対象として、ジストニアの病態解明を目的とし、かつ、ニューロフィードバックのターゲットとなりうる健常群との脳活動の違いを検証した。音楽家のジストニアは、楽器演奏時に異常な筋収縮が見られる神経疾患であるが、安静時機能結合に関して健常音楽家との差異があるかを検証した。さらに、安静時機能結合と音楽スキルとの関連についても検証を行った。ジストニアに罹患した音楽家21名、健常音楽家34名の安静時機能結合を全脳で比較したところ、音楽家のジストニアは課題特異的な疾患であるにも関わらず、基底核安静時機能結合ネットワークが被殻の右前部において群間で有意差があった。さらに、健常音楽家では、音楽スキルの一種である打鍵間隔の正確さと基底核の機能的結合の強さに有意な相関が見られたが、ジストニアに罹患した音楽家ではその相関関係が見られなかった。これらの知見は、音楽家のジストニアの病態解明の一助となると考えられる。
