Outline of Final Research Achievements

First, in order to clarify what kind of ethical beliefs affect moral distress (MD) experienced by the care manager (CM), we re-analyzed the previous research. As a result, ethical beliefs were divided into 1)for self and 2)for others. 1)The former included 1.role as a care manager, 2.role as an organization person, 3.evaluation from others. 2)The latter were; clients should be this way; families should be this way; service providers should be this way; belonging organizations should be this way; and humans should be this way. Next, while showing these concepts as support tools, as a result of implementing a support program including lectures and case studies for incumbent care managers, there is a possibility that it may lead to self-awareness and externalization of the ethical beliefs of CM itself.

Academic Significance and Societal Importance of the Research Achievements

在宅療養の場においては、包括的かつ継続的な在宅医療・介護の提供を行うため、利用者の疾病を理解し、複雑な社会保障制度を利用しながら、多様な社会資源をマネジメントする能力を持つケアマネジャー(以下CM)の育成が急務である。多様な価値体系の只中において否応なく押し寄せる利用者やその家族の信念・価値観に翻弄されながら、日本のCMはどのような信念・価値観あるいは倫理的ビリーフを大事にしているのか解明することは重要である。さらにCMの自尊心の低下や回避行動を最小限にするため、CM自身の信念や価値観あるいは倫理的ビリーフを自己覚知するような、専門職としてのスキルアップを図る研修や教育が求められている。
