Research Abstract |
The purpose of the present study was to determine the relationship between periodontal status and panoramic radiographic measures (cortical shape and fractal dimension of the mandible) in postmenopausal women with osteoporosis, and compare the periodontal status of them who use bisphosphonate therapy with those who do not. The population studied, which was part of a prospective bisphosphonate interventional study, consisted of 109 postmenopausal patients with osteoporosis (mean±SD, 67.8±6.3 years). Periodontal assessments at baseline included present teeth, alveolar bone loss (ABL) and community periodontal index (CPI). Mandibular cortical shape was categorized into one of three groups (normal, mildly to moderately eroded or severely eroded) on panoramic radiographs. Fractal dimension (FD) was also measured at periapical sites between canine and premolar of the mandible. Bone mineral density (BMD) of the lumbar spine was assessed by dual energy X-ray absorptiometry. Periodontal parameters between 33 women who use bisphosphonate therapy and 31 women who do not were compared. The mean spine BMD was 0.656±0.075g/cm^2(64.9±7.4% of young adult mean). The mean number of teeth present, mean ABL and FD value were 20.2±8.1, 25.7±9.8% and 1.589±0.034, respectively. The number of subjects with periodontitis (CPI codes 3 or 4), severe attachment loss (AL codes 2, 3 or 4) and eroded mandibular inferior cortex were 103(98.1 %), 67(63.8%) and 97(89.0%). The bisphosphonate therapy inhibited the percentage of teeth with CPI code 3 or 4(P < 0.01) and increased FD value (P < 0.10). These results indicate that postmenopausal osteoporosis may be associated with eroded mandibular cortex and generalized advanced periodontal destruction. Osteoporotic women using bisphosphonate therapy show significantly lower probing depths and greater FD. These observations suggest that bisphosphonate therapy in osteoporotic women may play a beneficial role in periodontal status.
