PETを用いた慢性脳虚血における経時的acetazolamide反応性

Research Abstract

平成17年度はPETを用いて、平成16年度に検査施行した正常ボランティア10例のデータを基に(各パラメーターの結果の平均±2SDを正常範囲とした)、平成16年度に引き続き脳主幹動脈閉塞性病変をもつ患者25例(2年間で合計50例)に対して、安静時脳血流量(CBF)、CO_2負荷後CBF、aectazolamide負荷後5分、15分、30分のCBFを測定、同時に脳酸素摂取率(OEF)、脳酸素代謝量(CMRO_2)も測定した。その結果、1)OEF、CMRO_2とも正常範囲の症例ではacetazolamide負荷後5分、15分と著明なCBF増加を認め、負荷後30分後15分後と比してむしろCBFの低下を認めた。2)OEF亢進、CMRO_2の症例では、負荷後5分のCBFが安静時CBFと比して低下(盗血現象)を認めたが、その後15分、30分と徐々にCBFの増加を認めた。3)OEF正常、CMRO_2低下の症例では負荷後5分、15分、30分と経時的に緩徐なCBF増加を認めた。またCO_2負荷時のCBFはacetazolamide負荷5分後のCBFと最も有意な相関が認められた。以上の結果から、1)日常臨床で汎用されているSPECTのみを用いることで、酸素代謝を推測することが可能。2)acetazolamide負荷後の至適測定時期は負荷後5分後。3)acetazolamide負荷後の経時的な脳血流の変化を測定する事が出来れば、脳酸素代謝、神経活性を見る事が可能であることが示唆された。
