| Outline of Final Research Achievements |
By using a peer review system of medical records which we have developed, we planned to investigate how patient care differs between doctors trained in university hospitals (UHs) and community hospitals (CHs). As a preliminary step, we conducted a nation-wide survey regarding outpatient independent medical care performed by senior residents. We sent questionnaires to 579 hospitals (114 UHs and 465 CHs). Responses were obtained from 283 facilities (61 UHs, 222 CHs) (response rate 48.9%). The percentages of outpatient independent medical care on weekdays were high in general internal medicine (GIM) (78.0%), 50-60% in gastroenterology, cardiology, etc., and low in neurology, rheumatology, and hematology (50% or less). The percentages of independent medical care on weekdays were significantly higher in CHs than in UHs in all departments except GIM.

| Academic Significance and Societal Importance of the Research Achievements |
日本の医学教育において、インプット評価（どのような教育が施されているか）・アウトプット評価（どのような能力が獲得されたか）はなされているが、アウトカム評価（その後どのような診療を行っているか）はなされていない。我々は、アウトカム評価法としてのカルテピアレビューシステムの開発を行ってきた。今回は、この方法を用いた「初期研修を大学病院と市中病院で行った場合のその後の診療の違い」の解析を目標に、前段階として全国アンケートを実施した。多くの診療科の50～60％で3年目専攻医が独立診療していることが明らかになった。近年、日本の臨床研修においても外来研修の重要性が強調されてきており、その影響が伺えた。
