Outline of Annual Research Achievements

本研究では，積極的治療が終了した終末期がん患者および家族の意思決定を支援する心理療法プログラムを開発することを目指している。本研究の目的は，終末期の療養先選択に焦点を当て，家族との意思の不一致が精神的苦痛に及ぼす影響を検討すること（研究1)，家族との意思の不一致に介入するプログラム（PL-FACT）を開発すること（研究2），PL-FACTの実施群と非実施群で精神的苦痛の差を検討すること（研究3），の3点である。平成29年度は研究1について，終末期がん患者およびその家族を対象にインタビュー調査を実施した。2016年11月1日から2017年9月30日までの期間で該当となった124名（患者62名，家族62名）のうち，身体・精神・認知機能の問題等により参加困難と判断された者，同意の得られなかった者を除いた22名（患者11名，家族11名）が中間解析の対象となった。これらの対象者においては，療養先に関する患者と家族の意思の不一致が見られるペアが約半数であり，こうした不一致を抱える患者のQOLは一致している患者と比較して低かった。2017年10月の日本サイコオンコロジー学会においては，終末期医療における家族間の意思決定について幅広く知識を得るとともに、本研究の成果を報告した。
なお，本研究の対象となる終末期がん患者は，転院，急変，死亡などで参加困難となる症例も多く，リクルートには難渋した。また残念ながら研究代表者の研究実施施設からの退職に伴い，継続が困難となった。したがって，2018年3月をもってリクルート終了し，本研究の成果について論文執筆中である。
