身体パフォーマンステストを応用した認知機能評価法の開発

Outline of Annual Research Achievements

本研究の目的は、身体パフォーマンステストを応用することで高齢者の認知機能を評価するための方法を開発することである。2017年度の研究内容および成果は以下の通りである。
１、6種類の身体パフォーマンステスト（握力、開眼片足立ち時間、5回椅子立ち上がり時間、timed up and go、5 m通常歩行時間、ペグ移動テスト）およびファイブ・コグによる認知機能の測定をベースライン時とその3年後に実施した169名のデータを解析した。その結果、5回椅子立ち上がり時間、timed up and go、ペグ移動テストは認知機能と横断的な関連性を示すこと、5 m通常歩行時間およびペグ移動テストの変化量（パフォーマンスの変化量）と認知機能スコアの変化量が縦断的な関連性を示すことが示唆された。これらの結果を総合し、手指の巧緻性が認知機能と強く関連する可能性を見出した。
２、身体パフォーマンステストの結果と基本属性から簡易的に高齢者の認知機能を推定する方法を検討した。その結果、どのような認知機能（どの認知機能評価法の結果）を推定するかにより、身体パフォーマンステストの結果と基本属性のどの指標を組み合わせるかが異なるものの、いずれの場合でも手指の巧緻性を評価する身体パフォーマンステストは重要な指標となることが示唆された。
３、ペグ移動テストとトレイルメイキングテストを組み合わせたトレイルメイキンペグテストをおこなうための機器を試作した。新たな機器はすべてコンピュータ制御であり、表示部を電光掲示に変更したが、これまで測定で使用していた妥当性と信頼性が検証済みの旧型（アナログ型）と同等の認知機能評価機器となることが示唆された。
