Outline of Annual Research Achievements

妊娠高血圧腎症(preeclampsia, PE)は妊娠によって発症する蛋白尿を伴う高血圧であり、母体 ・児の周産期死亡の主な原因である。PE患者で血液凝固が亢進し、増悪因子であることが示唆されているが、その意義は十分に検討されていない。活性化した凝固因子はプロテアーゼ活性化受容体2 (PAR2)を介して炎症を誘導し、様々な組織傷害に関与している。そこでPEにおけるPAR2の役割を解明するため、以下の検討を行った。
i) 子宮動脈結紮PEモデルの検討
我々は妊娠マウスの子宮動脈を結紮することで、PE様の表現型を示すことを報告した(Fushima T, Oe Y, et al. PLoS One. 2016)。本研究では妊娠アウトカムの評価に適したICR系統マウスで解析するため、5世代以上バッククロス行い、あらたなPAR2欠損マウスを作成した。膣栓確認後14.5日目に子宮動脈を結紮し、18.5日にサンプルを回収し胎仔・胎盤重量、胎盤組織評価、血管新生関連因子の発現を評価した。PAR2欠損により胎盤重量が低下し、胎仔発育不全が増悪した。胎盤において、ラビリンス層の面積が有意に低下しており,胎盤形成不全を伴っていた。胎盤における血管新生関連因子や内皮関連マーカーが低下しており、PAR2が妊娠において血管保護的であることが明らかになった。
ii) VEGF中和抗体投与PEモデルでの検討
VEGF中和抗体とPAR2欠損マウスを組み合わせPE病態にける腎障害を解析した。PAR2欠損により、蛋白尿が増悪し、糸球体血管内皮マーカー(CD31)、ポドサイトマーカー(Nephrin)が低下していた。電子顕微鏡的評価ではPAR2欠損により内皮fenestrationが消失し、Foot process effacementの所見も認めた。腎臓におけるVEGFなどの血管新生因子が減少していた。以上よりPAR2がVEGF阻害による腎障害に対しても保護的であることが明らかになった。
