Outline of Final Research Achievements |
This study showed that nocturnal hypertension, which increases cardiovascular risk, was frequently observed in 69% of patients with Parkinson's disease (PD). Although orthostatic hypotension (OH) has been reported as a risk factor for nocturnal hypertension, minimum systolic blood pressure during activity period and nocturia were better predictors of nocturnal hypertension than OH. Sleep-related factors other than nocturia did not necessarily show an association with nocturnal hypertension. Therefore, sleep disorders might be less likely to be directly involved in nocturnal hypertension of PD.

Academic Significance and Societal Importance of the Research Achievements |
過去の海外の検討では、PDの夜間高血圧(non-dipper型、reverse-dipper型)の頻度は31.6~92.3%と報告されており非常に幅がある。日本人67例を対象とした本研究では、69%の患者に夜間高血圧を認め、PDでは一般的にみられる所見であることが明らかになった。また、夜間高血圧に影響する要因として、夜間頻尿の関与が示唆されたことから、排尿障害への対策が夜間血圧調節の是正に有用かもしれない。
