| Outline of Final Research Achievements |
Firstly, we decided the adequate condition of pulsed water jet for open gynecologic surgery by experiment using pig. Secondly, we developed bladder-nerve stimulation device and pulsed water jet during laparoscopic surgery. The residual ability of bladder-nerve could be confirmed by this procedure. Finally, laparoscopic nerve-sparing radical hysterectomy for cadaver with Thiel’s method could be accomplished utilizing pulsed water jet.

婦人科がんに対する広汎子宮全摘術は骨盤内臓神経、神経叢、膀胱枝を損傷し、排尿障害を起こすことで患者のQOLを著しく損なう可能性のある術式である。神経を損傷せず一方で根治性を低下させないためには、神経を血管から正確に分離することが重要でさらに熱損傷を起こさないことが望ましい。パルスジェットメスの使用によりその手技を腹腔鏡下に実現することができ、婦人科がん患者の術後の排尿障害を大幅に減少させることが可能である。
