Outline of Annual Research Achievements

現在、日本でも子どもの貧困は深刻な問題であり、健康格差の広がりが懸念される。本研究は、子ども期の逆境体験とその後の健康状態との関連性を定量的に評価することを目的とした。申請者はこれまでに日本の大規模コホートデータを利用し、子ども期の虐待や経済的困難などの逆境体験が、50年以上経過した高齢期においても高次生活機能の低下と関連する可能性を示した。
最終年度は、今後の日本における子どもの貧困対策の推進に有用なエビデンスを提供するため、子ども期の逆境体験とその後の健康状態との関連性についてフィンランドとの国際比較研究を行った。日本に住む65歳以上の高齢者約14万人を対象とした日本老年学的評価研究(Japan Gerontological Evaluation Study, JAGES)のデータと、フィンランドのFinnish Public Sector(FPS)Studyの高齢者のデータを用いた。子ども期の逆境体験には両親の離婚、家庭内に恐怖のあること、貧困を用いた。健康指標には主観的健康観、がん、心疾患、脳卒中、糖尿病、喫煙、body mass index(BMI)を用いた。ロジスティック回帰分析を用いた。性別、年齢、教育歴、婚姻歴、就業の有無を調整した。
日本の高齢者の50%、フィンランドの高齢者の37%がひとつ以上の子ども期の逆境体験を経験していた。子ども期の逆境体験のあることと高齢期の健康を損なうことはどちらの国においても関連ししている可能性が示唆された。両国において子どもの貧困対策など子ども期の逆境体験を防ぐ取り組みが必要であると考えられる。
