Outline of Final Research Achievements

The kidney has a pivotal role for phosphate homeostasis. Glomerular filtered phosphate is reabsorbed in the proximal tubule. The phosphate transporters are expressed at the proximal epitherial cells, and they regulate phosphate reabsorption. Klotho is expressed in distal tubule and is involved in the regulation of phosphate metabolism, but its detailed mechanisms are unclear. In this study, we revealed a novel mechanism that klotho directly regulates phosphate transporter at proximal tubule via megalin.

Academic Significance and Societal Importance of the Research Achievements

慢性腎臓病患者や透析患者は心血管疾患を原因とする死亡リスクが高い。心血管疾患のリスク因子の1つが高リン血症であるが、現状の透析や食事療法、薬剤療法では適切なリン管理は困難である。また腎機能の低下を早期に発見するマーカーも確立されておらず、リン管理の早期介入が困難な原因となっている。
本研究では老化関連因子klothoがメガリンを介して近位尿細管で直接リン調節に関与する新規メカニズムの一端を明らかにし、新たな治療戦略の基盤となることが期待される。
