Research Abstract

ADAMTS13は超高分量von Willebrand factor(VWF)マルチマー(UL-VWFM)を分解するが、本酵素が低下した病態ではUL-VWFMが増加し血小板血栓形成傾向となり、諸臓器の微小循環障害が惹起される。本研究期間内で明らかにし得た点とその意義を列記する。
1.ADAMTS13の肝臓における産生細胞の同定:本酵素に対するモノクロナル抗体を作成し、免疫組織染色ならびにin situ hybridizationを行い、本酵素が肝星細胞で産生されることを明らかにした。
2.重症肝疾患における血漿ADAMTS13活性の動態とその意義
(1)肝硬変において、血漿ADAMTS13活性は肝の重症度が増すにつれて低下するが、末期には血栓性血小板減少性紫斑病に匹敵する程度にまで著減し、本酵素に対するIgG inhibitorを確認した。UL-VWFMは本酵素活性高度低下、高度腎障害、肝性昏睡例で検出し得た。(2)多臓器不全合併重症アルコール性肝炎および急性肝不全において、ADAMTS13活性の著減とUL-VWFMの著増が、肝障害の進展・多臓器不全発症と密に関連することを指摘した。本酵素活性の低下には炎症性サイトカインとinhibitorが関与することを見出した。(3)造血幹細胞移植後のveno-occlusive disease(VOD)は、ADAMTS13低値例で高率に出現するが、新鮮凍結血漿を予防投与するとVODの発症が阻止し得ることを確認した。さらに、生体肝移植後、早期の血小板減少にはADAMTS13活性の著減が密に関与し、虚血再還流障害、早期拒絶の-因となる可能性を指摘した。
以上、ADAMTS13が肝星細胞で産生され、本酵素活性の著減とUL-VWFMの著増が、進行した肝硬変、重症アルコール性肝炎、急性肝不全、VODおよび生体肝移植後早期の肝障害の発症ならびに多臓器不全の進展と密に関連することを明らかにした。これらの結果は、重症肝障害時における類洞内微小循環障害、多臓器不全に対する病態解明ならびに新たか治療戦略に繋がる可能性があり、その意義は大と考えられる。
