Research Abstract

[Objective] To evaluate the stem cells therapy for refractory skin ulcer, we attempted to graft the ulcer region cultured epithelial stem cells.
[Methods] The cell suspension of epithelial stem cells was applied on severe pressure ulcers with deep subcutaneous undermining ; i.e. cultured epithelial stem cells that did not reach until the confluence were harvested by using ordinary method after EDTA treatment for removing 3T3 feeder layer. The cultured epithelial stem cell, which was adjusted cell number, was applied undermining ulcer region with bio-gel (pat. pending). The epithelial sheet, which was cultured until confluent, was applied on the ordinary refractory skin ulcer. After the infection of ulcer wound was completely regulated by using various disinfectants, the cell therapy was performed. The epithelial stem cells applied on wound surface were dressed according to ordinary methods. The cell therapy was attempted once per a week during 4 weeks as far as circumstances permit.
[Results] All of these patients, who resisted against the ordinary therapy, obtained significantly reduction of wound area or reepithelization. The shortest period of the therapy was 2 times of treatment during 2 weeks but many patients reepithelized within 40 days by using this method.
[Discussion] At present, moist wound healing devise or cytokine such as FGF-2 are used for refractory skin ulcer. However, the wound healing process, which consist of the complicated mechanism, is not regulated by just one cytokine. Therefore, in case of these refractory skin ulcer patients, it was suggested that the release of various cytokines in epithelial stem cells ; i.e., good balance and coordination of cytokine network, was important such as the effective mechanism of cell therapy
