Research Abstract |
Object: The presence of regional metastatic lymph nodes in patients with oral cancer alter the treatment and the prognosis. Ultrasound, CT and MRI are useful tools for diagnosing of regional metastatic lymph nodes which is evaluated by morphological factors such as the shape, size and numbers. The accuracy of morphological factors is high but incomplete. The enhanced pattern of dynamic MRI correlated with intratumor angiogenesis, which corresponds closely to tumor growth. To provide additional information for diagnosis of regional lymph nodes, we investigated the relationship between the enhanced patterns acquired by dynamic MRI of metastatic lymph nodes and that of non-metastatic lymph nodes in oral squamous cell carcinoma (SCC). Methods: Twenty patients with primary oral SCC underwent dynamic MRI using three dimensional fast imaging with steady-state precession (FISP) sequence. The enhanced pattern of dynamic MRI of regional lymph nodes and morphological findings were acquired. The relationship between the enhanced pattern and morphological findings was analyzed. In the same patients, the relationship between the enhanced pattern of metastatic lymph nodes and non-metastatic lymph nodes was also analyzed. Results: Regarding the diagnosis for metastatic lymph nodes, morphological findings were more accurate than the enhanced pattern of dynamic MRI. There was no correlation between morphological findings and enhanced pattern of dynamic MRI. In the same patients, the metastatic lymph nodes were showed similar enhanced pattern to the primary site. On the other hand the enhanced pattern of non-metastatic lymph nodes was not similar to that of metastatic lymph and/or that of primary site. Conclusion: Dynamic MRI appeared to improve the evaluation of regional metastatic lymph nodes.
