Research Abstract |
Mass examination for osteoporosis is provided by many local authorities in Japan. This study was conducted to make it effective with limited resources and time in terms of both health care providers and users. For providers, we developed 1) new interview sheet which could boost effectiveness of education for preventing osteoporosis and 2) new guideline description which was useful for health care providers; public health nurses, dieticians and exercise instructors, to share the common understanding with each other and to ensure consistency when they gave users their advice. The computer-scored interview sheet was developed so that not only users could answer easily, but also health care providers could counsel easily. The items included in the interview sheet were selected based on the international risk factors reported by the WHO. by themselves. Many kinds of self-assessment tools had been developed in the world, but there were few tools specialized for Japanese women. In addition, in existing self-assessment tools, specificity became a victim of increasing sensitivity. So we entered the data collected using the interview sheet developed by us into Clementine, and tried to detect rule sets to explain BMD levels from self-assessment data, using CHAID algorithms. Finally, we made out the model to estimate the risk of bone loss by using few explanatory variables ; age, diet, weight, and height in their 20s. This model estimated the risk of bone loss with 86 percent precision, its sensitivity was 91 percent and specificity was 81 percent. To generalize these findings, it seemed to be necessary to accumulate data. However, it is suggested that this self-assessment tool could play a role to boost motivation for local residents to participate in the examination of bone density. Some of these findings were presented at International Osteoporosis Foundation World Congress on Osteoporosis 2006 (Toronto, Canada) and International Council for Nurses Conference 2007 (Yokohama, Japan).
