薬剤性有害事象の発生率、防止可能性に関する臨床疫学的研究

Research Abstract

本研究は日本における薬剤性有害事象や薬剤関連エラーに関する臨床疫学的データを科学的に収集し、それらの発生率や発生に関連する因子、患者側の認識を解析し、薬剤性有害事象や薬剤関連エラーの防止可能性や効果的な対策を分析するものである。また同時に、薬剤性有害事象や薬剤関連エラーの発生を定量的に予測する予測モデルを作成し、臨床現場や健康政策に還元することも目指している。平成19年度の研究では、全国規模の一般市民を対象とした横断研究について分析を行った。全国から無作為に抽出された15歳以上の一般市民2400人中、1109名が回答している。うち、106人(10%)の一般市民が、自分もしくは家族に対する診療において、何らかのエラーを経験していると回答していたが、実際に医原性有害事象もしくはエラーが関与していたのは、57件(5.1%)であった。その中で、薬剤性有害事象/薬剤関連エラーが最多であり、42%であった。これらの報告は、欧米からの報告と同等の結果であった。患者側の認識を分析すると、薬剤性有害事象/薬剤関連エラーの原因が、"薬剤に起因する"と考えるよりも、"医療従事者の能力に起因する"と考える一般市民が多いことが明らかとなった。また、9割以上の一般市民は最も責任がある関係者は医師である、と考えていた。
今後、平成17年度から19年度にかけての研究結果を元に、日本における薬剤性有害事象及び薬剤関連エラーの実態について、包括的な報告を行うと同時に、一般市民の視点も考慮した健康政策やアドボカシーを分析していく予定である。
