Research Abstract

内視鏡を用いたレーザ結石破砕が行われているが、Ho : YAGレーザ(波長2.1μm)は、高出力のため熱作用による生体組織への影響が問題である。Er : YAGレーザ光(波長2.94岬)は結石に吸収され易く、低出力で効率よく破砕できるが、有効な伝送路がなかった。
本研究は、Ho : YAGレーザ光とEr : YAGレーザ光を伝送し、従来では不可能であった衝撃波(Ho : YAGレーザ)と熱蒸散(Er : YAGレーザ)の複合効果破砕を可能とする高機能ファイバの提案を行い、そのための伝送路および先端素子を中空プアイバオプティクスで実現し、その製作法の確立と伝送システムの構築、そして結石破砕治療への応用を目的として、下記の研究を行った。
1.衝撃波・熱蒸散用中空ファイバ伝送装置の構築と評価
1本の中空ファイバ(長さ1.5m、内径700μm)で、Ho : YAGレーザ光とEr : YAGレーザ光を同時伝送するシステムの構築を行った。次にファイバの耐久性評価として、出射エネルギー150mJ/pulse、繰り返し周波数10HzのHo : YAGレーザ光(パルス幅250μs)とEr : YAGレーザ光(パルス幅300μs)を同時伝送し破砕実験に用い、破砕後のファイバに損傷はなく、十分使用に耐えうることを明らかにした。
2.衝撃波・熱蒸散破砕用中空ファイバの結石破砕への応用
結石破砕の基礎実験を行い、Er : YAGレーザ光の出射エネルギー150mJ/pulse程度にすると、破砕しにくい結石も比較的短時間で破砕できることを明らかにした。また高出力のEr : YAGレーザ光のみで破砕すると、結石移動が大きく破砕困難であるが、Er : YAGレーザ光の照射エネルギーを抑え、Ho : YAGレーザ光を同時伝送することにより、破砕効率は同程度で、結石移動を抑え効率よい破砕ができることを明らかにした。
