Research Abstract

本研究は腎不全進行におけるアルドステロンのMRを介した作用の関与について検討することを目的として立案された。平成17年度の実験結果より、1、従来MRは遠位尿細管や集合管にしか発現していないとされていたが、培養ラット腎糸球体メサンギウム細胞・ヒト近位尿細管細胞・ラット腎線維芽細胞などの細胞質にも蛋白レベルで強く発現していること、2、アルドステロンはMRを介して、数分以内でmitogen-activated protein(MAP)キナーゼを活性化させること、3、また、それらが尿細管での電解質の調節のみならず、各種細胞増殖や繊維化などに密接に関わっていること、4、アルドステロンはMRを介してNADPRオキシダーゼを活性化し、酸化ストレスを亢進させること、またこれは細胞質に存在しているNADPHオキシダーゼのコンポーネントであるp47phoxとp67phoxが細胞膜ヘトランスロケーションすることによって生じること、などを明らかにした。平成18年度ではさらに、1、アルドステロンはを介してRhoキナーゼを活性化し、ストレスファイバーを産生して細胞形態の変化に関与していること、2、アルドステロン慢性投与によって生じる各種臓器障害は、Rhoキナーゼ阻害薬によって軽減されること、3、ネラロコルチコイド受容体を介した種々の情報伝達機構が、DNA転写機構を介さない非ゲノム作用によって生じていること、4、このようなアルドステロン/MRの作用の一部に、アンジオテンシンIIのAT_1受容体が関わっていること、5、2型糖尿病性腎症ラットではMR拮抗薬が著名な腎保護効果を示すこと、などを証明した。このように、アルドステロンはMRを介して様々な腎障害作用を生じ、これを阻害することが新しい治療ターゲットになりうることが示唆された。
