Research Abstract

愛知県内の某自治体に勤務する35歳以上の男性職員で、2002年のベースライン(BL時)生活習慣アンケート及び健診成績のある4,317名から、がん、虚血性心疾患、脳卒中の既往(59名)、糖尿病、高血圧、高脂血症の治療歴(476名)、BL時食事アンケートから推定される摂取エネルギー量が500Kcal未満あるいは3500Kcal以上の者(181名)を除外した3,601名のうち、1,920名が2005年の追跡調査に参加した。このうち、BL時脂質異常(HDLC<40/TG≧150mg/dl)、空腹時高血糖(≧110mg/d)、血圧高値(≧130/85mmHg)のうち2個以上の異常を有さず(1,688名)、かつBL時アディポネクチン濃度の測定値のある1,248名を解析対象者とした。メタボリックシンドローム(MS)発症は2005年に腹囲が85cm異常で、脂質異常、空腹時高血糖、血圧高値のうち2つ以上有する者とした。3年間の追跡期間中96名(7.7%)にMSが発症した。BL時アディポネクチン濃度は非発症者で6.45μg/ml、発症者で5.31μg/mlであった(P<0.001,t-test)。BL時のアディポネクチン濃度の四分位(低い群からQ1〜Q4)を説明変数、3年後のMS発症を目的変数とした多重ロジスティック回帰分析から最もアディポネクチン濃度の高いQ4群を基準とするとQ1、Q2、Q3群のMS発症オッズ比(95%信頼区間)はそれぞれ3.06(1.41-6.61)、2.24(1.01-4.96、2.15(0.95-4.85)であった(年齢、BMI、HOMA-R、n3系脂肪酸摂取量、不飽和/飽和脂肪酸比、食物繊維摂取量、身体活動、喫煙、飲酒を補正)。
