| Outline of Final Research Achievements |
A multiple regression analysis was used data collected from 308 nurses involved in care for older people. The analysis showed that professional autonomy in nursing was related to the Sense of Coherence Measurement Scale (β = .313, p < .001), Scale of Nursing Practices for Elderly with Cognitive Impairment with the Aim of providing Person-centered Care in Acute Hospitals (β = .276, p < .001), and length of years working as a nurse (β = .138, p < .01) (R2 = .23).
This study provided 90 nursing staff with educational interventions related to decision-making support for people with dementia and management of physical complications for people with dementia. The results show the necessity to correctly assess the decision-making ability of older people under the care of multidisciplinary cooperation in a hospital. The findings suggest that certified nurses in dementia nursing need to cooperate with ward nursing teams to continuously assess the decision-making ability of older people.

| Academic Significance and Societal Importance of the Research Achievements |
介入したA町、I町の高齢化率は著しく高く、医療過疎という深刻な課題に直面していた。このような地域に認知症トータルケアプログラムを適用し、地域の認知症看護力の強化を目指した。その結果、医療過疎地域の認知症看護力を推進する看護職者の専門職性の向上に貢献した。
地域横断的看護力の向上のために、多職種協働チームと病棟看護師チームが連携し、高齢者の意思能力に関する情報共有と継続的なアセスメントを在宅に至るまで行っていく必要性が確認された。このことは、認知症者とその家族が住み慣れた地域で主体的に生活するために重要な視点であり、地域包括ケアシステムの充実に貢献する知見である。
