Outline of Final Research Achievements

We have developed the assessment method for adult congenital heart disease and surgery. At first, we developed a tool for comprehensive assessment of hemodynamics and cardiac function of the right and left ventricle, and we developed 4D flow MRI post processing software and scan protocol.
Then, we have applied in adult congenital heart disease cases who often have problems in the pulmonary valve and the right ventricular outflow tract. Flow energy loss is the sensitive parameter for the cardiac workload in these diseases, and pulmonary valve surgery prominently reduced the energy loss in these cases.
We have developed a 4D flow MRI post processing software and we have published several papers regarding hemodynamics assessment of the adult congenital heart disease cases before and after the surgery.

Academic Significance and Societal Importance of the Research Achievements

治療介入基準や診断、血行動態のアセスメントが極めて難しいとされ、エビデンスが枯渇していた成人先天性心疾患において3次元的に複雑な解剖であるにもかかわらず、系統的に左右心機能と体肺循環血行動態を評価し、問題点を明確にすることで手術適応、薬物療法の適応を明確にすることができた。
手術加療を行うことでエネルギー損失値の低下とともに心負荷が大きく軽減することが確認されたが、結果患者は運動耐容能が増大し、就労などに制限がなくなり質の高い生活ができるようになっており、若年患者での新たなエビデンスを発信したことが社会的に大きな意義をもたらしたと考えられた。
