Outline of Annual Research Achievements

本研究は、地磁気搭載慣性センサ、インソール型足圧センサ、レーザーレンジセンサ等の計測機器を用いて、変形性膝関節症（膝OA）の進行および関節痛の予後予測が可能な動的診断システムを開発し、そのシステムに準じたテーラーメードリハビリテーション介入が、膝OA進行および関節痛に与える影響を検証することを目的としている。
令和元年度では、医療機関において外来診療中の膝OA患者1名を対象とした、慣性センサ評価を併用したテーラーメードリハビリテーション介入の症例検討を行った。本症例検討においては、平成30年度に行ったデータ解析に加えて、新たなセンサ解析手法（Gaussian continuous wavelet transform）ならびに動画解析手法（OpenPose）を取り入れた。これらの解析手法を追加することで、リハビリテーション介入効果を多角的かつ視覚的に捉えることが可能となった。具体的には、外来リハビリテーション前後での左右脚の歩幅や立脚期時間を算出し、歩行中の膝関節角度などの変化を視覚的に提示したことで、患者へのフィードバックを従来よりも効果的に行うことができた。このシステムに基づいた6週間のリハビリテーションによって良好な治療経過を得ただけでなく、その治療メカニズムについて迫ることができた。
このような、センサを用いた定量的評価に準じたテーラーメードリハビリテーションはこれまで体系化されてこなかった。本症例報告は、今後のさらなる研究の火付け役になる可能性がある。これらの結果はJBJS Case Connectorに受理された。
