Outline of Annual Research Achievements

慢性炎症腸管粘膜に特徴的に蓄積する遺伝子異常の同定
慢性炎症粘膜に特徴的に蓄積する遺伝子異常をクローンレベルで捉えるため,潰瘍性大腸炎患者から提供を受けた慢性炎症粘膜より計76ラインのクローナルなオルガノイド株を樹立し,エキソーム解析を行った.その結果,まず慢性炎症に伴い遺伝子変異の総数は軽度であるが統計学的に有意に上昇し,この傾向は関連癌を合併した症例において顕著であることが判明した.さらに,慢性炎症粘膜には炎症シグナルに関連した遺伝子(NFKBIZ, PIGR, TRAF3IP2)に変異が認められた.変異遺伝子はIL-17シグナルを構築する主要蛋白をコードしていることから,慢性炎症粘膜にはIL-17シグナルに関連する遺伝子変異が特異的に蓄積すると考えられた.この現象の実証のため,樹立初期の多クローンからなる慢性炎症上皮オルガノイド111例および健常上皮オルガノイド32例を対象とし,IL-17シグナル関連遺伝子を含む炎症関連遺伝子パネルのターゲットシーケンスを行った.慢性炎症粘膜にはエキソームシーケンスによって同定された遺伝子変異が高頻度で認められ,IL-17シグナルに関連する他の遺伝子(IL17RA, ZC3H12A, NOS2)にも変異が特異的に蓄積することが判明した.
同定された遺伝子変異の機能的解析のため,CRISPR-Cas9システムを用いて正常大腸上皮オルガノイドの遺伝子編集を用った.IL-17Aは正常大腸上皮オルガノイドはNOS2を介したプログラム細胞死を誘導するが,いずれの遺伝子改変オルガノイドもこの反応に対する感受性が低下しており,IL-17A存在下でも生存可能であった.
以上の結果より,潰瘍性大腸炎患者における腸管病変では,慢性的な炎症暴露に伴いIL-17シグナルに関連する遺伝子変異を獲得したクローンが選択的に増殖することを実証した.
