超音波による難治性潰瘍の非侵襲的モニタリングシステムの研究開発

Outline of Annual Research Achievements

今年度は,これまでに開発してきた信号解析法を実装した超音波診断装置を用いて,臨床試験でのin-vivoデータ収集および解析を行った.臨床での患者数の制約から下肢健常およびリンパ浮腫症例(約60例)を主な対象とした.また,他の定性評価手法(シンチグラフィ法やインドシアニングリーン蛍光造影法)によるリンパ浮腫進行度も整理し,超音波による定量診断結果と比較した.
これまでに行ってきた正常および外科手術の対象となる重症度リンパ浮腫組織の性状理解に関して,摘出組織での検討も引き続き検討した.マクロな後方散乱波特性とミクロな音響特性および組織構造の関連性を同一指標で明らかにするために,後方散乱係数という物理量の実測値と理論モデルに基づく推定値を比較した.これらの比較により,数 mm領域のマクロスケールでの解析において,ミクロスケールの組織構造や物性を反映した組織性状評価が可能であるとの知見を得ることができ,これはリンパ浮腫における炎症や浮腫の質を定量評価できると推察できた.
さらに,昨年度から実施しているシステムの高速撮像化に関する実験的検討を引き続き行った.ファントムでの基礎検討から血管系を対象としたin-vivo実験まで一貫して検討した.超音波の送受信を複数の条件設定で行うなどし,本検討に最適な条件の検証を含めたアルゴリズムの考案を行った.
