Outline of Final Research Achievements |
Circulating TNF receptor (TNFR) levels predict all-cause mortality in hemodialysis patients, but not their ligands [TNFα, Progranulin (PGRN)]. Prednisolone treatment decreased circulating TNFR2 levels in addition to improvement of proteinuria and hematuria in patients with IgA nephropathy. Renal mRNA levels of TNFα and TNFR2 were increased, but their circulating levels decreased in high-fat diet-fed PGRN knockout mice compared with their wild type mice. PRGN may show both inflammatory and anti-inflammatory effects depending on organ such as kidney and adipose tissue.

Academic Significance and Societal Importance of the Research Achievements |
末期腎不全患者において、血中TNFR濃度が高値の患者では、臨床因子とは独立して生命予後予測に有用であることを明らかにした。これまで腎機能(GFR)、蛋白尿以外に腎臓病患者の予後予測することは困難であった。本研究結果により、血中TNFRは保存期腎臓病患者の腎予後のみならず、末期腎不全患者の生命予後も予測できることを明らかにした。 以上より、慢性腎臓病患者において本マーカーを測定することにより、患者のリスクを層別化することが可能となるため、臨床的意義は高いと思われる。
