| Outline of Final Research Achievements |
This study has clarified the effectiveness of care management and interprofessional work as a method for promoting collaboration between various professions in community-based integrated care system. A survey of Regional Comprehensive Support Centers nationwide revealed that care management and interprofessional work are factors that promote cooperation in community-based integrated care system.

| Academic Significance and Societal Importance of the Research Achievements |
これまで地域包括ケアシステムに関する研究は、医療・介護情報の共有化（ICT活用やツール開発）や社会資源の充実（ネットワーク化、組織化など）、制度・政策の整備といったハード面に注目されてきており、本研究のように多職種連携の方法というソフト面の研究は少ない。さらに本研究は、多職種連携の方法の検討のみならず、連携を促進する取り組みの明確化に至った点で、新たな視点を含んだ研究であるといえる。その成果は、地域特性に応じて多種多様などと曖昧にされてきた多職種連携の方法の基盤を提示するものになると考えられ、これまでの研究が示してこなかった新たな知見を得たと考える。
