Outline of Final Research Achievements

The purpose of this study was to clarify the current status and issues of nurse collaboration and cooperation between home visit nursing stations that support elderly people with dementia treated at home and nursing homes or day care service centers that provide respite care to family caregivers, and to examine effective collaboration between the facilities. It was found that collaboration among nurses is implemented, albeit infrequently, through the use of contact notes and telephone calls, and that they feel the need for such collaboration among each other. It was also clear that they wish to further improve the efficiency of communication tools and increase opportunities for interaction in the community in order to build face-to-face relationships. The fact that information is often shared through other professions suggests that mutual understanding with these other professions is also necessary to promote direct collaboration among nurses.

Academic Significance and Societal Importance of the Research Achievements

認知症高齢者を支援する訪問看護ステーションとレスパイトケア施設との看護師連携の現状と課題が明らかとなり、効果的な連携方法が検討されることで、認知症高齢者の認知症症状が悪化することなく、効果的なショートステイや通所介護の提供が可能となる。これにより家族介護者への効果的なレスパイトケアの提供が可能となり、安定した介護の提供は、認知症高齢者が安心・安全・安楽に地域で生活することへ貢献する。認知症ケアにおける地域包括ケアシステムの推進、ひいては医療費の削減に貢献するものと考える。
