Research Abstract

【目的】ヒトの内臓感覚を作る脳部位はどこで、どのように制御されるのか?この問題はこれまでほとんど追求されていない。これを解決することが本研究の目的である。内臓知覚過敏と情動異常の双方を示す病態が過敏性腸症候群(IBS)である。その脳機能が解明できれば、内臓知覚から情動形成に至る経路が解明されうる。IBSでは催眠暗示により変化する内臓知覚時の局所脳賦活パターンが健常と異なるという仮説を検証した。【方法】対象は健常対照者(n=12)とIBS患者(n=12)である。直腸にバロスタットバッグを挿入し、催眠状態に導入し、鎮痛、中性、過痛の3つの催眠暗示を与えた後に大腸伸展刺激(40mmHg)を加えた。刺激と同時に、H_2^<15>Oを右正中肘静脈に静注し、positron emission tomography(PET)にて局所脳血流量を撮像した。【結果】対照者では、鎮痛暗示+刺激によって右腹外側前頭前野(VLPFC)とsubgenualACC(sgACC)が賦活された(p<0.005)。これに対して、IBS患者においては、鎮痛暗示+刺激によって右VLPFCと島が賦活された(p<0.005)。更に、同一の操作を受けた対照者に比べ、IBS患者においては、過痛暗示+刺激で、左扁桃体と後部帯状回の活性が有意に高まった(P<0.005)。鎮痛暗示+刺激では、対照者に比してIBS患者において右傍海馬領域が有意に高く(P<0.005)、sgACCは有意に低かった(P<0.005)。【考察と結論】IBS患者では催眠による内臓知覚時の局所脳賦活パターンが健常者とは異なるという仮説が支持された。これらの錯反応を正常化する脳科学的手法を開発することができれば、IBSを克服し、かつ、治療法のない難治性の内臓痛を制御できるようになる可能性がある。
