Research Abstract

ヒト二足歩行の中枢性制御には,大脳基底核-視床-運動皮質系と基底核-脳幹系の2つの系が重要であると考えられている。このモデルの検証のため,加齢性白質障害(ARWMC)を対照とした多角的イメージング研究を行い,さらにモデルの改良を目的として,運動に伴う体性感覚入力が脳活動に及ぼす影響を検討した。(1)ARWMC患者(歩行障害群11名,非障害群9名)において,歩行分析,シングルフォトン断層像(SPECT)による歩行時脳血流評価,拡散テンソル画像(DTI)による白質線維走行健全性の評価を行った。歩行分析では,歩行障害群における歩行速度減少,両脚支持期の増加,歩隔の増加が明らかになった。歩行時脳血流の評価では,補足運動野,一次運動野,一次体性感覚野,視覚野,脳幹,小脳の活動増加を認め,健常高齢者やパーキンソン病にっいて報告した所見と一致した。歩行障害群と非歩行障害群の比較を行うと,歩行障害群において,歩行時の補足運動野,視床,基底核の活動低下が示された。これら領域の活動は,歩行分析から得られた両脚支持期パラメータとの相関を示した。DTI解析では,歩行障害群において,補足運動野,視床や基底核周囲の白質に異常が認められ,これらの領域を結ぶ白質線維の障害に伴う,大脳基底核-視床-運動皮質連関の異常が,歩行障害の原因となっているという考えを支持した。(2)運動に伴う筋活動による感覚入力と,運動関連脳活動の関係を知るため,筋電図と機能的MRIの同時測定を行い,一次運動野に対する一定強度の経頭蓋磁気刺激(TMS)により誘発される筋電図(MEP)の積分値と,MRI信号変化の関係を検討した。予備的解析の結果では,補足運動野と腹側運動前野にMEP積分値と相関する脳活動が認められた。運動に伴って運動関連領野に見られる活動の解釈には注意が必要であり,引き続き検討が必要であることが示唆された。
