Research Abstract

膜乳化法という先駆的なナノテクノロジー技術を応用し、癌血管選択的なインテリジェントデリバリーシステムを開発するための基礎研究を行った。我々が現在までに進めている中性子捕捉療法(BNCT)はボロン原子と熱中性子の核反応により細胞障害を生じる(^<10>B+^1n→^7Li+^4He(α)+2.31MeV)。その反応飛呈は約10μmであり、ボロン原子を腫瘍選択的に集積できれば、周囲の正常組織を傷害せずに癌細胞を選択的に破壊することが出来る。我々は、難治癌のひとつである肝細胞癌に対して中性子捕捉療法を応用すべく、VX-2ウサギ肝腫瘍モデルに対し、ボロンキャリアーとしてWOWエマルジョンを用いることにより癌組織選択的にボロン原子を集積することに成功した。Na_2^<10>B_<12>H_<11>SH(BSH)水溶液とLipiodolのエマルションをシラス多孔質ガラスフィルター(SPG膜)を通しボロン封入WOWエマルションを作製した。このエマルションをVX-2ウサギ肝腫瘍モデルに対して固有肝動脈より投与し、ICP-Masspectroscopy腫瘍組織および正常肝組織におけるボロン濃度を測定し、さらに組織学的検討を行った。ボロン封入WOWエマルションを肝動注1, 3, 7日後の腫瘍組織におけるボロン濃度はそれぞれ141ppm、61ppm、28ppmであり対照群のボロンLipiodol混和液の肝動注1, 3, 7日後の腫瘍組織におけるボロン濃度はそれぞれ58ppm、24ppm、9ppmであった。 組織学的にWOWエマルション投与群では腫瘍組織においてLipiodol混和液よりのサイズの小さい均一な脂肪滴を認めた。さらに、電顕像においてWOWエマルション投与群では腫瘍細胞内にWOWエマルションの取り込み像を認めた。以上の結果より、肝臓癌において癌選択的にボロン原子を集積させるにはWOWエマルションがより優れている事がわかり、中性子捕捉療法におけるボロンキャリアーとして応用が可能である。
原発性肝癌の治療においては肝切除が行える症例は約30%と少なく、術前・後に施行される肝動注化学療法では抗癌剤とリピオドールを混和して注入するが、この手法では抗癌剤は容易にリピオドールと分離し腫瘍内に貯留していない現状であり、標準治療の確立が急務である。我々は2段階膜乳化法というWOWエマルションのサイズをマイクロ~ナノオーダーまで個別のサイジング可能な最新の手法を用いて、臨床応用を目指しGLPレベルのエピルビシン封入WOWエマルションを作製し前臨床試験を施行中である。南九州に存在するシラスを用いて熱および酸処理することによりシラス多孔質ガラスフィルター(SPG膜)を開発した。このSPG膜を用いて、エピルビシン封入WOWエマルションを作製した。我々が作製したWOWエマルションは約70μmに均一なサイジングができており現在までに約10ヶ月間安定性を確認している。このエピルビシン封入WOWエマルションをウサギVX-2肝腫瘍モデルに肝動注した場合に著明な腫瘍増殖抑制効果を認めた。HE染色・脂肪染色により腫瘍組織におけるリピオドールの沈着を認め、電子顕微鏡像において腫瘍細胞内にWOWエマルションの集積像を認めることもできた。腫瘍細胞へのエピルビシンの集積はWOWエマルションを用いて増加しEndocytosisの機序も考えられる。このWOWエマルションを用いての肝臓癌に対する標準治療への応用が期待できる。
