Research Abstract

膵癌の前癌病変の1つと考えられているIPMN (Intraductal Papillary Mucinous Neoplasm)切除症例を亜型分類し、レーザーマイクロダイセクション法を用いて腫瘍細胞のDNA を抽出し、K-ras、B-raf、PI3Kの変異解析および免疫組織染色により、p53異常集積、p16、SMAD4の発現低下、ERK のリン酸化の頻度を検討した。その結果、悪性度の高いとされる腸型亜型を呈するIPMN ではp53遺伝子変異の頻度、悪性度の低いとされる胃型亜型を呈するIPMN ではK-ras 遺伝子変異の頻度が高かった。通常型膵癌ではK-ras 遺伝子変異が極めて高率であることを考慮すると、胃型亜型では何らかの要因が加わり通常型膵癌の発生に関連し、腸型亜型では発癌過程の後期に起こるとされるp53 遺伝子変異によりIPMN 由来癌の発生に関与するという異なる発癌過程が考えられた。
