Research Abstract

本研究は、不全心筋がイオンチャネルの発現低下による活動電位延長とCa^<2+>過負荷による心筋傷害の悪循環に陥っていることに着目し、発現の低下したイオンチャネルの再生によって心不全を制御できるか否かを検討することを目的とした。心不全モデルとしてはLewisラットにブタ心筋ミオシンを感作させて作成する自己免疫性心筋炎ラット(EAM)を用い、その急性期(感作後14-21日)に一過性外向きK+チャネル(Ito)の発現低下を主機序とする活動電位延長が起こっていることを確認した。予備研究として、活動電位を短縮する薬物(KATPチャネル開口薬:ニコランジル・メキシレチン)の投与実験を行い、急性期の活動電位短縮と慢性期の心機能改善を確認した。Ito遺伝子(Kv4.2, KChiP2)導入のためのプラスミド(ベクターはPires-hrGFP-1a)を作成し、NIH3T3細胞、oocyteへのItoチャネルのtransfectを確認できたが、Gene-gunによるラット心筋表面への打ち込みでは十分なチャネル発現を得ることが出来ず、心筋の活動電位を変化させるには至らなかった。予備研究において、急性期の活動電位を修飾することで心不全を治療する可能性を示すことが出来たが、Gene-gunによる遺伝子移入には限界があり、今後はチャネルをtransfectした細胞の移植など、チャネル再生のための新たな方法論を検討する必要があると考えられた
