We have been studying the nature of slit diaphragm (SD) of podocyte that is accepted to play a critical role for maintaining the barrier function of glomerular capillary wall. However, the molecular composition of the SD was not fully understood yet. In this project, we intended to identify the functional molecule of the SD. In the first year of this project (2006), to identify the molecules which are involved in the development of proteinuria, the subtraction assay was performed using normal and proteinuric glomerular materials. Synaptic vesicle-protein 2B (SV2B) is detected to be clearly downregulated in the proteinuric glomeruli. In the second year (2007), the expression and the function of SV2B in podocyte were analyzed. The analyses with siRNA techniques showed that SV2B play a role for maintaining the proper localization of CD2AP, a critical molecule of the SD. We also observed that the expression of ephrin B1 is clearly decreased at the very early stage of disease before the onset of proteinuria. The findings suggest that ephrin B1 play a critical role for maintaining the barrier function of the glomerular capillary wall. We published these findings as manuscripts in the top journals of Nephrology field, Kidney International and Journal of American Society of Nephrology. We reported some important results at the annual meeting of the American Society of Nephrology held in San Francisco (2007). We are convinced that the results obtained in this project contributed to the better understanding the pathogenic mechanism of proteinuria and to establishment for the novel therapy.
